Please note that the costs of the voluntary examination and testing for Sars-CoV-2 in the travel test facilities are not covered by the health insurance and you have to pay them yourself.
I consent to medical notifications and certificates being sent electronically using my patient ID, mobile phone number and e-mail address or, in the case of an external laboratory, my data being transmitted to the same.
In the event of proof of a notifiable disease according to the German Infection Protection Act, the doctor and laboratory are legally obliged to report the proof, including your data, to the responsible health authority.
I have registered for a voluntary Sars-CoV-2 test. I am aware that if the test is positive, I will not be able to continue my journey and will not be allowed to stay in the transit area of ¿¿the airport. I am also aware that if the test is positive, I will have to go into quarantine at home (currently up to 14 days) due to the regulations applicable in Germany. The costs incurred for this, e.g. B. Accommodation costs with hotel and meals as well as follow-up tests will be borne by me, to which I hereby undertake. The authorities or Fraport AG will not bear the costs.
